Job summary

Leeds Community Healthcare NHS Trust is seeking a Senior Nurse to join the Neighbourhood Response Team in Leeds. The role focuses on rapid assessment and treatment in patients’ homes, supporting independence and timely discharge from hospital.

You will lead and develop services, work with multidisciplinary colleagues, and contribute to ongoing innovation in urgent community care across Leeds. Flexible shifts are available across 7am–6pm, seven days a week.

Qualifications

• Must hold current UK nursing registration (NMC).
• Experience in community-based urgent care is desirable.
• Strong communication and teamwork skills.

Responsibilities

• Provide rapid assessment, intervention and treatment within 0–2h, 2–4h and 4–48h timeframes.
• Collaborate with Community Matrons, District Nurses and other teams to deliver seamless care.
• Demonstrate sound clinical decision-making and leadership.
• Support service development, innovation and continuous improvement.
• Promote patient safety, experience and governance within patients' homes.
