Surrey Downs Health & Care is expanding its Recovery At Home service to provide timely care and support that facilitates discharge from hospital and onward assessment in a community setting.

The Senior Rehab Support Worker will oversee care and rehabilitation delivery, coordinate services, and support a multidisciplinary team to ensure patient-centered planning and timely access to services.

Responsabilidades

• Consult with patients to determine their needs and develop care plans.
• Coordinate patient-care services and liaise with colleagues to evaluate interventions.
• Work with a multidisciplinary team, community teams and Social Prescribers as part of the Recovery At Home service.
• Support other professionals in coordinating all key activity including access to services, advice and information, and timely, efficient, patient-centered health and care planning.
• Develop skills and abilities within clinical teams to better support the client group.
