Job summary

Surrey Downs Health & Care in Epsom is looking for a proactive Rehabilitation Support Worker to join the Recovery At Home service. You will provide care, support and rehabilitation in community settings to facilitate timely discharges from hospital and prevent admissions, following a care plan and reporting to the Daily RAH Coordinator and Senior on duty.

You will work with the MDT, use risk assessment tools, and develop skills to deliver evidence-based care with dignity and person-centered

Qualifications

• Full UK driving license and access to a vehicle insured for work.
• Deliver clinical and therapeutic care per patient care plans.
• Support assessment, planning, and delivery of care under supervision.
• Identify need for referrals and follow through appropriately.
• Provide handovers to team leads and the wider MDT.
• Promote health education per local and national policies.
• Support patients’ independence and daily living activities.

Responsibilities

• Deliver clinical and therapeutic care per patient care plans.
