Job summary

Practice Plus Group is seeking a Senior Primary Care Nurse to lead a nurse-led service across 2-3 shifts weekly, including nights and a weekend per month. You will manage a multi-disciplinary team, undertake medication rounds, assess patients and respond to emergencies, while delivering compassionate, evidence-based care tailored to individual needs.

The role emphasises leadership, CPD, and collaboration with partners to ensure safe, high-quality healthcare in secure environments.

Responsibilities

  • Lead a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Provide outstanding evidence-based care to patients, informed by feedback and what matters to them
  • Deliver non-judgemental care that makes a real difference
  • Inspire excellence by forging strong links with partners and stakeholders and provide a trusted, safe, and proud service
  • Promote best practice, clinical supervision, evidence-based care and continuous quality improvement
  • Share knowledge and expertise to ensure safe care and strong teamwork

Job description

25 hours a week working 6.45am-7.30pm over 2-3 shifts per week over 7 days/nights. This will include 1 or 2 nights a month and one full weekend a month. Night shift is 7.30pm-6.45am., As a Senior Primary Care Nurse, your responsibilities will vary. You will

  • Lead a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
  • Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
