Job summary

One Health Lewisham seeks an Advanced Nurse Practitioner to provide home-based care for housebound patients across the borough. You will assess, diagnose and treat undifferentiated conditions, order investigations, and refer when necessary in collaboration with senior medical staff.

Applicants must be an independent prescriber with Full NMC registration, UK practice qualification, and EMIS experience. A car and clean licence are essential for doorstep visits.

Qualifications

• NMC registration is required to practise in the UK.
• Prescribing qualification is essential.
• Minimum 2 years' general practice experience is required.

Responsibilities

• Assess and manage complex undifferentiated presentations.
• Order investigations (blood, urine tests, ECGs, imaging) and refer as needed.
• Prescribe and review medications as an independent prescriber.
• Provide clinical cover across services as needed.
• Communicate with patients, carers and colleagues with clear consent.
