Job summary

Thurrock Council is seeking a Senior Practitioner for the Urgent Community Response Team (UCRT). You will provide professional leadership, undertake complex assessments, and coordinate rapid response to prevent hospital admissions, while supporting safe discharges.

You’ll mentor colleagues, manage a dynamic, 7-day caseload, and work with health partners to ensure integrated, person-centred services. This role requires safeguarding expertise and a commitment to high-quality care in the community,

Qualifications

• Relevant Social Work qualification (degree) and registration with Social Work England.
• DBS check enhanced level with Adult Barred List is required.

Responsibilities

• Provide professional leadership and oversight within the Urgent Community Response Team, supporting high-quality, person-centred practice.
• Undertake complex assessments and make informed decisions to manage risk and ensure safe, timely interventions.
• Coordinate and deliver rapid response support to prevent hospital admissions and facilitate safe discharges.
• Supervise and support team members, offering guidance, mentoring and reflective practice.
• Work collaboratively with health partners and other agencies to ensure integrated and effective service delivery.
• Manage a dynamic caseload with a 7-day service, prioritising urgent needs.
