Job summary

Wirral Community Health & Care NHS Foundation Trust is seeking a Clinical Team Leader for the Urgent Community Response Service in Wirral. You will provide operational and clinical leadership to a multidisciplinary team delivering rapid assessment, treatment, rehabilitation and care to patients in their own homes, within the Community Integrated Response Team.

Qualifications

• Registered with the Health and Care Professions Council (HCPC) or relevant body.
• Degree or Diploma in a relevant clinical area.
• Evidence of portfolio of knowledge, skills and professional development.
• MSC in Advanced Practice.
• V300 independent nurse prescriber qualification.

Responsibilities

• Provide high‑quality, patient‑centred assessment, treatment and care in patients' homes and community settings.
• Alternating into the Shift Leader role to oversee daily operational management of the caseload.
• Act as a clinical expert and support junior staff with supervision and mentorship.
• Support service improvements, audits, governance processes and staff wellbeing.
• Deputise for the Service Lead as required and represent the service in meetings.
• Help develop innovative pathways and models of care within community care provision.
