Job summary
Wirral Community Health and Care NHS Foundation Trust is seeking a Clinical Team Leader within the Urgent Community Response (UCR) Service in Wirral. Qualifications
- Registered with the Health care professionals council or relevant professional body
- Degree or Diploma in relevant clinical area
- Evidence of portfolio of knowledge, skills and professional development
- MSC in Advanced Practice (Desirable)
- V300 independent nurse prescriber qualification (Desirable)
Responsibilities
- Providing high-quality, patient-centred assessment, treatment and care to patients in their homes and community settings
- Overseeing daily operational management of caseload and triage of referrals
- Acting as a clinical expert and support for the team
- Supervising and mentoring junior staff to ensure safe patient care
- Leading service improvements, audits and governance to enhance outcomes
- Contributing to workforce management including sickness, appraisals and wellbeing
- Deputising for the Service Lead and representing the service in meetings
- Supporting development of innovative pathways and models of care for integrated community provision
