Job summary

Suffolk County Council is seeking a Senior Practitioner for its Flexible Care and Support Team (West). This role combines frontline social work with professional leadership to deliver person-centred support for adults, families and carers.

You will manage complex cases and guide colleagues, shaping services that promote independence and wellbeing. You will work with health partners, community organisations and local networks, ensuring timely, outcome-focused care.

Qualifications

• Significant experience working with adults with complex needs (disability, mental health, dementia).
• Excellent assessment, analysis and decision-making with a strengths-based approach.
• Experience leading complex casework and supporting colleagues.
• Strong communication and relationship-building across health, social care and partners.

Responsibilities

• Manage a small caseload of complex cases with expert assessment and risk management.
• Provide leadership and supervision to frontline professionals within the team.
• Support service improvement and ensure high standards of professional practice.
• Build partnerships across health, social care, housing and voluntary sector to deliver integrated services.
