Job summary

Herefordshire Council is seeking a Senior Practitioner to join the South Locality Team on a permanent 37-hour contract. You will hold a small caseload and work within the locality management team to support performance and lead practice.

You should be a qualified social worker registered with Social Work England and bring strong asset-based, outcome-focused assessment skills. The role requires travel across Herefordshire, with salary £42,123 to £46,579 plus a welcome payment and LGPS membership.

Qualifications

• Registered with Social Work England.
• Hold a Social Work qualification.
• At least three years post-qualified experience in Adult Social Care.
• Experienced in line management and supervision.
• Experienced in undertaking person centred, asset based, outcome focused assessment, care management and review.

Responsibilities

• Manage a small caseload and support performance as part of locality management.
• Make evidence-based decisions and lead practice within the team.
• Work with diverse service users across health and social care needs (mental health, disability, frailty, safeguarding).
• Contribute to development of specialisms and professional development.
