Job summary

Herefordshire Council is seeking a Senior Practitioner to join the Herefordshire Community Wellbeing Care Act Assessment Team on a permanent 37-hour week. You will manage a small caseload, support evidence-based decision making, and contribute to performance and practice development within the management group.

You must be Social Work England registered, hold a Social Work qualification, have 3+ years post-qualifying experience in Adult Social Care, and be able to travel across Herefordshire.

Qualifications

• Registered with Social Work England.
• Hold a Social Work qualification.
• At least three years post qualified experience in Adult Social Care.
• Experienced in line management and supervision.
• Experienced in undertaking person centred, asset based, outcome focused assessment, care management and review.

Responsibilities

• Discharge planning and Care Act assessments & support planning.
• Build and develop identified specialisms and maintain professional development.
• Lead practice and supervise team ensuring outcome-focused care.
• Travel across Herefordshire as required to support core responsibilities.
