Qualifications

• Qualified social worker with strong knowledge of adult social care.

Responsibilities

• Carry out short-term assessments and support planning for service users in Herefordshire.
• Support service users who have recently left hospital through the Discharge to Assess process.
• Apply strong case management skills and effective decision‑making throughout assessment and review.
• Provide supervision and support to staff on a day-to-day basis and undertake formal and informal supervision as required.
• Take responsibility for team duty allocations, triaging and sign-offs of assessments and workflows.
• Apply positive risk-taking and evidence-based practice to ensure safe and effective outcomes.
