North Cumbria Integrated Care NHS Foundation Trust in the east Cumbria region is seeking a Band 6 nurse in the Specialist Palliative Care team, serving the community. The nurse runs a clinical caseload with autonomous practice alongside CNSs and the Consultant in Palliative Medicine, with flexible to meet service needs.

The role involves managing patient and family needs, delivering specialist palliative care, and contributing to education of colleagues.

Qualificações

• Degree-level education or equivalent experience.
• Post-registration qualification in palliative care or related field.
• Current NMC registration as a Registered Nurse (Level 1).
• Experience assessing and managing patients with complex palliative care needs.
• Experience supporting education, supervision or development of colleagues.

Responsabilidades

• Manage a clinical caseload, prioritising and responding to fluctuating patient and family needs.
• Deliver person-centred specialist palliative care to patients and families across east Cumbria.
• Give specialist advice, assessment and support to patients with complex palliative care needs, working with CNSs and the consultant.
• Assess, manage and review patients, supporting symptom control and end-of-life care.
• Work with hospital teams and community services to deliver safe, coordinated care.
• Contribute to education, training and mentorship of colleagues.
• Keep accurate clinical records in line with professional standards.
