You will work across Norfolk’s North, South and Central areas, with a base aligned to your Place team, delivering expert palliative and end-of-life care in patients’ homes and care facilities.

The role involves triage, home visits, and a dedicated palliative care advice line for professionals, patients and families.

Responsibilities

• Triage across palliative care caseloads.
• Community visiting to patients in their homes.
• Palliative care advice line for professionals, patients and families.
• Link with wider Specialist Palliative Care Service, including the consultant-led inpatient unit, psychology and day therapies.
• Audit, education/teaching and research involvement.
