Imperial College Healthcare NHS Trust in London is seeking a Band 7 Palliative Care Clinical Nurse Specialist for a fixed-term 6-month contract. The post offers the opportunity to deliver expert nursing care for patients and families, working within a multidisciplinary team to maintain high standards and ensure personalised care.

You will conduct comprehensive palliative assessments, contribute to clinical decisions, and ensure care plans are implemented and evaluated in line with Trust

Responsibilities

• Provide expert nursing advice and support to deliver high-quality, individualized palliative care for patients and families or carers.
• Work collaboratively with the multidisciplinary team to ensure care is aligned with local and national standards.
• Share a caseload with other Specialist Palliative Care team members and maintain effective communication with patients, families, carers and healthcare professionals.
• Undertake comprehensive palliative care assessments, contributing to clinical decision-making and prioritisation of care.
• Identify and address physical, psychological, social, cultural and spiritual needs to ensure holistic, high-quality nursing care.
• Refer and discuss with other team members to optimise patient care and ensure care plans are implemented and evaluated according to guidelines.
• Ensure staffing is in line with Trust policy and resources are managed efficiently.
• Champion a culture of learning and quality improvement within the team and an ambassador for palliative and end-of-life care across the Trust.
