Job summary

Mid Cheshire Hospitals NHS Foundation Trust is recruiting a Specialist Palliative Care CNS (Band 7) to join the Community Specialist Palliative Care team in Crewe. You will work with palliative care medics, district nurses and GPs to deliver highly specialist care to patients and families in the CCICP footprint.

This part-time role offers shared caseloads, clinical leadership, and opportunities for audit, education and research within a supportive MDT framework across multiple sites.

Qualifications

Registered nurse with adult nursing degree and NMC registration.
Masters level qualification or equivalent preferred.
Prescribing qualification (V300) is desirable.
Experience in palliative or community care is essential.
Strong communication and teaching skills expected.

Responsibilities

Lead and deliver specialist palliative care in the community setting.
Coordinate care with MDT including medics, nurses and GP teams.
Provide holistic assessments and management plans for complex cases.
Participate in audits, education and research activities.
