Dorset County Hospital NHS Foundation Trust is seeking a highly motivated palliative and end of life care Clinical Nurse Specialist to join our hospital-based specialist service.

The role requires leadership in delivering a 7-day nursing service, with collaboration across MDT and community partners to provide holistic, evidence-based care. You will contribute to education and development of the palliative and end of life care team.

Responsabilités

• Provide a 7-day-a-week specialist nursing service as part of the hospital palliative care team.
• Review, develop and facilitate palliative care services with the MDT to ensure holistic, evidence-based management.
• Collaborate with community, hospital and voluntary sector colleagues.
• Provide specialist nursing leadership in line with local and national guidelines.
• Be responsible for own strategic planning and direction of the palliative care service.
• Deliver holistic assessment of needs and implement patient-centred care.
• Maintain up-to-date knowledge of developments in palliative care.
