Job summary

Nottinghamshire Healthcare NHS Foundation Trust is seeking a Senior Occupational Therapist (Pathway Determiner) for our Community Therapy Services in the Nottingham West area. The role involves working in patients' homes and coordinating care with a multidisciplinary team to promote recovery and independence.

You will join a developing integrated service providing clinical triage, rehabilitation, falls management, palliative support, and efficient discharge planning.

Qualifications

• Full UK driving licence and vehicle for business use (required).
• Experience of working in community health settings is desirable.

Responsibilities

• Work autonomously within a multidisciplinary team to coordinate community-based care.
• Visit patients in their own homes to maximise independence, safety and quality of life.
• Support timely discharges from hospital as part of integrated community services.
