Job summary

You will assess referrals, develop treatment plans, promote independence, and collaborate with MDTs to support patients across diverse conditions.

Qualifications

• HCPC registration as an Occupational Therapist.
• Experience in discharge planning and reablement.
• Skilled in community-based care and hospital-to-home transitions.

Responsibilities

• Assess new service user referrals and care/support needs.
• Develop, implement and evaluate treatment/care plans.
• Promote independence and prevent unnecessary hospital admissions.
• Collaborate with multidisciplinary and multi-agency teams.
• Support safe hospital discharge and improve health and wellbeing.
