Job summary

Nottinghamshire County Council is seeking an Advanced Occupational Therapy Practitioner in Adult Social Care to supervise a dedicated team, assess needs, and coordinate outcomes that promote independence and choice for service users. You will lead and mentor staff, identify reablement opportunities, engage with multidisciplinary teams, and contribute to safeguarding investigations and discharge planning.

Qualifications

• HCPC registration required.
• Two years’ post-qualification OT experience, preferably with complex cases.
• Evidence of CPD (Continuing Professional Development).
• Knowledge of community care and adult social care legislation.

Responsibilities

• Assess needs through comprehensive OT evaluations.
• Coordinate outcomes with a person-centred approach.
• Lead and mentor staff, fostering collaboration and support.
• Promote independence by identifying reablement opportunities and community resources.
• Monitor service provision to ensure effectiveness and satisfaction.
• Contribute to safeguarding investigations and discharge planning.
• Engage with multidisciplinary teams to enhance service delivery.
