Job summary

Wye Valley NHS Trust is seeking a Senior Occupational Therapist (Band 6) to join the Discharge to Assess (D2A) service in Hereford. You will assess patients in their own homes after hospital discharge and develop personalised rehabilitation plans to promote independence and safe discharge.

You will work within a MDT, coordinating with therapy teams, social care and primary care to optimise patient outcomes and reduce length of stay.

Qualifications

• Degree in Occupational Therapy.
• HCPC Registration.
• Experience in NHS or equivalent settings.
• Experience with home-based assessments and rehab planning.

Responsibilities

• Conduct holistic OT assessments in patients' own homes post-discharge.
• Assess activities of daily living, mobility, cognition and risks.
• Develop personalised rehabilitation and reablement plans to maximise independence.
• Arrange equipment, adaptations and support services; liaise with health and social care teams.
• Support MDT working to ensure safe discharge and continuity of care.
• Provide supervision to junior staff and contribute to service development.
