Job summary

Your Healthcare CIC is recruiting a Senior Nurse Practitioner (Band 7) to join the Urgent Care & Support Service in Kingston. You will deliver advanced clinical assessment, autonomous decision‑making, and independent prescribing to residents across nursing, residential and learning disability care homes.

You will lead complex case reviews, support multidisciplinary teams and drive quality improvements, governance, and staff development within the care home sector.

Qualifications

• Significant post‑registration experience in community services including assessment, management and delivery of care to residents with complex needs.
• Autonomous assessment, clinical decision‑making, prescribing and management of complex clinical cases.
• Experience leading or contributing to clinical audit, service evaluation and quality improvement.

Responsibilities

• Provide advanced clinical care to residents with complex needs in care home settings.
• Lead proactive case management and admission avoidance initiatives to reduce hospital attendances.
• Participate in clinical governance, service evaluation and quality improvement using outcome data.
