Job summary

Wirral University Teaching Hospital NHS Foundation Trust is recruiting a Band 7 Senior Nurse Practitioner to join the newly commissioned Unscheduled Care Coordination Hub (UCCH). You will provide clinical leadership, undertake urgent care triage and coordinate management to improve access to community services and reduce hospital admissions.

You will work across Cheshire and Warrington with NWAS and system partners, applying established pathways to support patients at first contact and guide

Qualifications

• Experience leading multidisciplinary teams in urgent care.
• Ability to triage and assess acutely unwell patients.
• Capable of autonomous decision making in first-point-of-contact scenarios.
• Experience with community-based care coordination.

Responsibilities

• Lead within the Unscheduled Care Co-ordination Hub per UCR framework.
• Provide clinical assessment of acutely unwell patients and support frailty initiatives.
• Advise on treatment options aligned with community nursing and social care.
• Support telephone triage and determine appropriate clinical pathways.
