• Coordinate care for a complex caseload of service users.
• Formulate recovery-focused care and risk management plans with service users and relatives.
• Carry out mental health assessments.
• Offer advice and support to service users and carers.
• Act as initial point of contact for older people facing mental health challenges.

Responsibilities

• Supervise Band 5, Band 4 and Band 3 staff where appropriate.
• Complete home visits.
• Work under supervision of Deputy Team Manager or Team Manager.
