Job summary

Caerphilly County Borough Council invites applications for a Senior Practitioner within the South Community Mental Health Team. The post focuses on assessment and care management for adults with mental health needs, requiring registration with Social Care Wales and relevant post-qualifying qualifications.

You’ll work across the borough, liaising with carers and professionals, with a strong emphasis on safeguarding and safe discharge planning. A comprehensive benefit package accompanies the role.

Qualifications

Diploma or Degree in Social Work or equivalent.
Post-Qualifying Awards for Consolidating Practice and Enabling Practice or equivalent.
Registered Social Worker with Social Care Wales; OMF/APMH training if needed.

Responsibilities

Deliver assessment and care management for adults 18+ with mental health needs.
Work with carers, professionals and teams and when needed use initiative.
Support safe discharge and safeguarding of vulnerable service users.
Travel across the borough using a driving licence to visit clients and attend meetings.
