Job summary

Derbyshire Community Health Services NHS Foundation Trust in Derbyshire is seeking a Senior Community Nurse for a 12-month secondment to cover maternity leave. You will lead the rapid response team, manage caseloads and support service development within the integrated community team in Swadlincote.

Qualifications

• Registered nurse with current NMC registration.
• Nursing degree or equivalent relevant experience.
• Student mentorship experience or capability.
• Leadership development training or programme exposure.

Responsibilities

• Provide clinical leadership to the rapid response team.
• Coordinate and manage a busy triage list.
• Support workload allocation and service delivery across the team.
• Act as a role model for high-quality nursing care and professional practice.
• Supervise, teach and develop junior staff and students.
• Collaborate with GPs, therapists, social care and partner organisations.
• Support clinical governance, audit and quality improvement initiatives.
• Promote independence and self-management for patients at home and in the community.
