Job summary

Birmingham Community Healthcare NHS Foundation Trust is seeking an experienced Children’s Nurse to work in the community, providing safe, expert care to children and families in their homes. You will assess needs, plan care, and deliver treatment with multidisciplinary support, ensuring high standards of documentation and clinical governance.

The role requires leadership in the absence of a Team Leader, teaching skills for students and staff, and a commitment to ongoing professional development

Qualifications

• Demonstrates knowledge relating to child protection issues and procedures.
• Evidence of assessment skills and use of nursing process and nurse models.
• Knowledge of NMC Code of Conduct and scope of professional care.
• Knowledge of NHS and social care legislative changes affecting primary care.
• Knowledge and application of evidence-based practice in Children's Community Nursing.
• Ability to teach relatives, patients and staff in self-care and nursing practice.
• Excellent written documentation skills; concise and legible records.
• Awareness of diversity in family needs, culture and religion.
• Time and resource management in a busy community setting.
• Excellent communication skills with multidisciplinary teams.
• Family-centered care and meeting family expectations.
• Problem-solving with diverse information to implement actions.
• Ability to plan, prioritise and organise care efficiently.

Responsibilities

• Work in accordance with NMC Code and Scope of Professional Practice.
• Provide care in clients' homes as a lone practitioner with telephone support.
• Deliver calm, supportive communication for complex information.
• Communicate with multidisciplinary team to ensure best care for referred child.
• Carry out risk assessments prior to home visits to ensure staff safety.
• Collaborate with families to deliver personalised care respecting beliefs.
• Formulate management and treatment plans using clinical reasoning.
• Provide symptom management and monitor deterioration using available prescribing where allowed.
• Promote detailed care plans to prevent unnecessary admissions and reduce hospital stay.
• Ensure holistic care using an agreed nursing model and robust record keeping.
• Liaise with acute services, primary care and other agencies to facilitate early discharge.
• Advise leadership on concerns regarding patient care and partner services.
• Maintain accurate, contemporaneous records in RiO and paper systems.
• Offer flexible services to meet changing needs of children and families.
