Job summary

Leeds Community Healthcare NHS Trust is seeking a Band 6 Health Case Manager to join the Health Case Management Service which supports adults across Leeds, including those funded via NHS Fast Track and Continuing Healthcare. The post involves coordinating care, leading the service and supervising staff within a fast-paced environment.

The role requires professional registration with a relevant field (nursing, OT or social work) and a BIA qualification or commitment to training.

Qualifications

Professional registration in nursing, occupational therapy or social work.
Best Interest Assessor (BIA) qualification or willingness to undertake BIA training.
Experience of working with adults and their carers in hospital and/or community settings.
Highly organized, able to work independently and as part of a multidisciplinary team.

Responsibilities

Manage a complex caseload.
Collaborate with MDTs including ICB, Neighbourhood Team, LTHT and legal team.
Coordinate day-to-day tasks and referrals.
Provide a Lead role for the service and supervise/mentor staff.
Develop skills in safeguarding, personal health budgets, and clinical supervision.
Complete DOLIC assessments and other legal tasks such as S21A challenges.
