Job summary

Leeds Community Healthcare NHS Trust in Leeds is seeking a Band 6 Health Case Manager to join the Health Case Management Service. The role involves leading complex caseloads for adults, working with MDTs, and coordinating referrals to deliver person-centred care within hospital and community settings.

Qualifications

• Professional registration is essential and a Best Interest Assessor (BIA) qualification or the aspiration to undertake BIA training.
• Experience of working with adults and their carers in hospital or community settings.
• Excellent organisational and problem-solving skills; able to work independently and as part of a team.
• Motivated, organised, and able to manage competing demands in a fast-paced environment.

Responsibilities

• Manage a complex caseload.
• Work alongside MDT including, ICB, Neighbourhood Team, LTHT and legal team.
• Support with coordinating of day-to-day tasks and referrals.
• To provide a Lead role for the service.
• Supervise and mentor staff.
• Have opportunities to be develop skills in safeguarding, personal health budgets, clinical supervisions, deprivation of liberty, mental capacity, clinical nurse assessments for end-of-life pathways.
• Complete DOLIC assessments and other legal work such as S21A challenges.

Job description

The Health Case Management Service provides acase management function for the population of Leeds (18 years and above) whoare eligible for NHS Fast Track and Continuing Healthcare funding. Patients canbe in the community or requiring support from acute settings.

The service operates 365 days a year, over 7days, Monday to Friday 08:00 to 18:00 and Saturday and Sunday 09:00 to 17:00,Bank Holiday hours 09:00 to 17:00.

A fantastic and challenging opportunity hasarisen in the Health Case Management Service. We are looking for a dynamic,self-motivated Band 6 Health Case Manager (HCM) to join our team. We welcomeapplications from health professionals from a wide range of backgrounds includingNurses, Occupational Therapists and Social Workers. It would be essential forapplicants to have a professional registration and a Best Interest Assessor(BIA) qualification or have the aspiration to undertake the BIA training.

You will have experience of working with adults and theircarers within a hospital and /or community setting. You will have excellentorganisational skills and be able to work on your own independently andcreatively using a problem-solving approach. It is essential that you aremotivated, organised, a team player and enjoy the buzz of working in a fastpaced and dynamic environment. Thesuccessful candidate will be able to work well under pressure, managingcompeting demands and be able to prioritise according to service requirements.

Main duties of the job

The appointed applicant will:-

The allocated Health Case Managers (HCM)central philosophy and approach is placing the individual at the centre andproactively supporting patients. The HCM will collaboratively work alongsidepatients to promote choice and control over their care and support. The HCM will have oversight of the personscare and offer options and services to meet an individual's health and socialneeds through available resources to promote quality and cost-effectiveoutcomes.
