Job summary

NHS Lanarkshire is seeking an Advanced Practitioner in Frailty (Band 7) to join its frailty teams at University Hospital Hairmyres and University Hospital Monklands. The role focuses on early assessment, rehabilitation, and safe discharge planning for older people living with frailty.

You will work in a 7-day service with weekends and public holidays, with options for full or part-time hours. You’ll join a collaborative team and contribute to service development and patient outcomes.

Qualifications

• Registered professional with valid NMC/HCPC/GPHC registration.
• Masters level qualification or working towards.
• Postgraduate Diploma in Advanced Clinical Practice.
• Comprehensive knowledge of clinical guidelines and standards.
• Advanced specialist knowledge and experience in relevant conditions and practice.

Responsibilities

• Autonomously assesses, diagnoses and treats individuals with a range of conditions across lifespan.
• Utilise advanced knowledge to assess, plan, implement and evaluate programmes of care.
• Practice within Professional Codes of Conduct.
• Conduct evidence based practice in line with guidelines and standards.
• Manage own time across clinical, educational and administrative duties.
• Develop and participate in research with multidisciplinary and academic partners.
