Job summary

Lancashire Teaching Hospital NHS Foundation Trust is seeking an Advanced Nurse/Clinical Practitioner (ACP/ANP) to join a Primary Care Network multidisciplinary team. The role focuses on advanced clinical assessment, diagnosis, treatment planning and prescribing for frail care home residents with complex health needs.

You will work closely with GPs, care home staff and the wider team to reduce hospital admissions, enhance continuity of care and support personalised care planning.

Responsibilities

• Advanced clinical assessment of frail care home residents with complex healthcare needs.
• Triage patients and manage undifferentiated presentations within scope of practice.
• Prescribe safely and in line with national guidance and local care pathways.
• Refer to local services and specialist pathways as appropriate.
• Lead structured care home clinical review sessions and MDT meetings.
