Calderdale and Huddersfield NHS Foundation Trust is seeking an Advanced Clinical Practitioner for Acute Frailty and Urgent Community Response on a fixed-term basis. The post holder will work autonomously, making high-level clinical decisions to assess and manage frailty in acute and community settings.

The appointee will rotate across CHFT Urgent Community Response and relevant teams to provide care closer to home and to support the transformation of pathways for frailty care.

Qualifikationen

• Assess individuals, families and carers holistically, using a range of methods including history taking and physical assessment.
• Use clinical judgement to decide referrals for investigations and diagnoses.
• Make treatment decisions including prescribing medicines where appropriate.
• Ensure continuity of care and evaluate outcomes with patients and carers.
