Maywood Healthcare Centre is seeking an experienced GP to join the Neighbourhood Frailty Team within Bognor Coastal Alliance PCN. You will lead clinical assessments, care planning and multidisciplinary collaboration to support frail patients in the community.

The role involves up to six sessions per week on a 12-month fixed-term contract, with flexibility to share sessions among GPs. Emphasis on proactive, personalised care and reducing avoidable hospital admissions.

Qualifications

• Diploma in Geriatric Medicine or postgraduate qualification in frailty or palliative care.
• 2+ years post-CCT in UK GP with experience in frailty and multimorbidity.
• Full GMC registration with a licence to practise (NHS England)

Responsabilités

• Provide senior clinical assessment in clinics, care homes and patients' homes.
• Lead multidisciplinary meetings and coordinate care with INT/PCN colleagues.
• Develop personalised care plans to promote independence and reduce admissions.
• Support pathways with community nursing, social care and therapy services.
• Offer clinical supervision and contribute to governance and quality improvement.
