Job summary
South Downs Health and Care Ltd in Eastbourne is seeking a Salaried GP with an Interest in Frailty to join the Integrated Neighbourhood Team. You will deliver senior clinical care to frail, multimorbid patients in community settings, home visits and care homes, collaborating with Frailty Practitioners and GP practices to prevent unnecessary admissions.

Qualifications
- Essential: At least 2 years post-CCT in UK general practice; managing complex older patients and multimorbidity; medicines management
- Desirable: Frailty, care home, community or intermediate care roles; MDT leadership; clinical supervision or education
- Essential: GMC registration with licence to practise
Responsibilities
- Provide senior clinical assessment in clinics, homes, and care settings
- Carry out CGA and structured medication reviews with the team
- Develop digital personalised care and support plans and TEPs
- Lead anticipatory care planning and risk stratification
- Support safe discharge and continuity of care
- Provide clinical leadership to the INT and mentor colleagues
- Collaborate with GP practices, geriatricians, social care and community teams
- Attend MDT meetings and contribute to governance and service improvement
