Job summary

Procare Community Services is seeking a highly motivated District Nurse to join our clinically led Community Nursing Team. You will visit patients in their homes and care homes, manage a caseload across 1–2 GP surgeries, and provide holistic assessments and health promotion.

The role requires a District Nursing Qualification, NMC registration, and an ability to lead and support staff and students within a collaborative MDT environment.

Qualifications

• Registered Level 1 Nurse with current NMC registration.
• Experience in community nursing and MDT environments.
• Full UK driving licence with access to a vehicle for business use.

Responsibilities

• Maintain strong relationships with GPs, practice nurses, Social Services and hospital colleagues.
• Attend Gold Standard Framework and MDT meetings.
• Ensure regular meetings with GP and Practice Nurse colleagues.
• Manage a caseload across 1–2 GP surgeries with complex health needs.
