Connection Support is recruiting a Senior Complex Needs Outreach Worker to join the Rough Sleeper Team in Buckinghamshire. The role focuses on helping people experiencing homelessness, poor mental health, and complex needs access housing, health services, and supportive interventions.

You will work proactively to engage reluctant individuals, develop tailored support plans, and coordinate with partner agencies to secure positive outcomes and independence.

Formación

• Experience supporting people with mental health needs and complex vulnerabilities.
• Ability to engage with individuals reluctant to access services.
• Experience completing holistic assessments, risk assessments and creating support plans.

Responsabilidades

• Delivering assertive outreach to people sleeping rough or in emergency accommodation.
• Building trusting relationships with individuals who may be disengaged from services.
• Supporting people to access mental and physical health services.
• Completing holistic assessments, risk assessments, and tailored support plans.
• Coordinating with partner agencies to achieve positive outcomes.
• Verifying rough sleepers 2–3 times per week.
