Job summary

Connection Support in Buckinghamshire is seeking a Senior Complex Needs Outreach Worker for our Rough Sleeper Team. You will deliver assertive outreach, engage with people sleeping rough or with complex needs, and help them access housing, health and support services.

You will assess needs, develop tailored support plans, coordinate with partner agencies, and travel across the county using a vehicle. The role is full-time, fixed-term until 30 June 2027, with regular travel and office bases in

Qualifications

• Experience of supporting people with mental health needs and complex vulnerabilities.
• Experience of working with people who are homeless, sleeping rough, or facing multiple and complex disadvantages.
• The ability to build positive, professional relationships and motivate people to work towards their goals.
• Experience of completing assessments, support plans, risk assessments, and maintaining accurate records.
• Strong communication and partnership-working skills, with the confidence to work alongside a range of statutory and voluntary agencies.
• Good IT skills, including the use of Microsoft Office and case management systems.
• Willingness to support with verifications 2–3 times per week.

Responsibilities

• Delivering assertive outreach to people sleeping rough or in emergency accommodation.
• Building trusting relationships with individuals who may be disengaged from services.
• Supporting people to rapidly access and engage with mental and physical health services.
• Completing holistic assessments, risk assessments, and support plans tailored to individual needs.
• Working collaboratively with partner agencies to coordinate support and achieve positive outcomes.
• Verifying rough sleepers 2–3 times per week.
