Leeds Community Healthcare NHS Trust is recruiting a Triage Clinician to join the Neighbourhood Team Triage Hub. The role focuses on clinical triage, prioritising patient need, and delivering timely advice in a high-volume community setting.

You will work with neighbourhood team colleagues, GPs and partner services to ensure patients receive the right care at the right time, including escalation of urgent or complex cases and maintaining accurate clinical records.

Competenze

• Clinician with experience in clinical triage and prioritisation.
• Experience working in community nursing care.
• Ability to provide signposting and safety-netting.

Mansioni

• Clinically triage referrals to community nursing services according to clinical need and risk.
• Provide timely clinical advice, signposting, and safety-netting to patients, carers and professionals.
