Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking two Advanced Therapy Support Workers to join community teams in Chesterfield and North East Derbyshire. The posts are 1.0 WTE (37.5 hours) in Chesterfield and 0.8 WTE (30 hours) in North East Derbyshire, working across patients’ homes and care homes.

In this role you will assess, plan and review patient care, liaise with the wider therapy and nursing teams, and maintain health records in line with Trust policies.

Qualifications

• Experience in rehabilitation, physiotherapy or occupational therapy support work desirable.

Responsibilities

• Conduct initial assessments and reviews to meet therapy goals.
• Create patient-led care plans and monitor progress.
• Liaise with Occupational Therapists, Physiotherapists, Nursing teams and Social Services.
• Maintain accurate health records using System One/TPP in line with Trust policies.
• Work as part of a weekend roster to respond to Rapid Therapy visits.
