Job summary

Derbyshire Community Health Services in Derbyshire is recruiting Advanced Therapy Support Workers to join community teams in Chesterfield and North East Derbyshire, delivering rehabilitation in patients' homes and care homes.

The roles include initial assessments, care plan development, and liaison with therapists and nursing teams. You will work within Trust policies and support devices, with weekend responses and travel across the locality required.

Qualifications

• Experience in rehabilitation, physiotherapy or occupational therapy support work desirable.

Responsibilities

• Initial assessment and reviews of patients, creating patient-led care plans and meeting therapy goals in line with competencies and liaison with other specialist services within the Trust.
• Work independently and adhere to Trust Lone Working policies and communicate information to patients and carers.
• Maintain and record accurate health records using System One/TPP in line with Derbyshire Community Health Services policies.
• Organise and prioritise day-to-day work tasks and delegate tasks to colleagues and support administrative tasks.
