Job summary

Derbyshire Community Health Services NHS Foundation Trust (DCHS) seeks two Advanced Therapy Support Workers to join our community teams in Chesterfield and North East Derbyshire. The posts are 1.0 WTE (37.5 hours) in Chesterfield and 0.8 WTE (30 hours) in North East Derbyshire, with a weekend roster for Rapid Therapy visits.

You will work alongside Occupational Therapists, Physiotherapists and Nursing staff to deliver rehabilitation at patients’ homes and care homes, developing patient-led care

Qualifications

• Willingness to undertake further recognised training applicable to the role.
• Good standard of education including English Language and Maths GCSE or equivalent.
• NVQ Level 4 in health or social care or equivalent experience.
• Assistant Practitioner Qualification or equivalent.

Responsibilities

• Initial assessment and reviews of patients and create patient led care plans.
• Meet therapy goals in line with competencies and liaise with specialist services.
• Communicate information to patients and carers about health needs and treatment plans.
• Report changes in patient condition and update care team records.
• Maintain health records using System One/TPP; support service development.
