Job summary

The Methodist Church in Britain is seeking a Band 6 Senior Community Physiotherapist to join the North Hampshire Community Therapy Team. The role involves domiciliary rehabilitation in patients' homes, managing a caseload within a multidisciplinary team, and delivering evidence-based treatments to prevent hospital admissions.

Part-time position (22.5 hours/week) with opportunities to develop leadership skills.

Qualifications

• Professional qualification as a physiotherapist or occupational therapist.
• Level 6 degree plus postgraduate specialist training, experience or short courses to masters-equivalent level.
• Good command of English, written and verbal.

Responsibilities

• Assess patients with acute, chronic and complex multi-system conditions using evidence-based practice.
• Manage your own caseload while working within the multidisciplinary team.
• Triage patients and liaise with the correct services.
• Deliver treatment programmes, or direct interventions through support workers and other MDT members.
• Facilitate safe discharge, including risk assessment, ordering equipment and advising patients and carers.
