Somerset NHS Foundation Trust is seeking a Band 6 (or developmental Band 5-6) Community Physiotherapist to join the Community Rehabilitation Service in the South Somerset area (Chard, South Petherton, Crewkerne).

You will manage a varied community caseload including frailty, amputee rehab, neurology, complex long-term conditions, orthopaedics and respiratory, working across community hospitals and with a multidisciplinary team to support patient-centered care and rehabilitation.

Qualifications

• BSc Hons Degree in Physiotherapy and HCPC registration are essential.
• Postgraduate experience in relevant field is desirable.
• Experience across community rehabilitation settings preferred.

Responsabilités

• Provide high quality patient-centered care and specialist rehabilitation in the community.
• Supervise and support Band 5 Therapists and rehab staff; act as Team Lead when required.
