Responsibilities

• Manage a geographically defined caseload delivering symptom assessment and psychosocial support.
• Provide specialist palliative care advice to meet individual care needs.
• Apply referral guidelines and undertake levels of intervention as agreed.
• Demonstrate clinical decision-making and monitor care standards via audit, practice supervision, and teaching.
• Participate in a seven-day rota to cover community needs.
