Job summary

Dorothy House Community Palliative Care Team is seeking a Community Nurse Specialist (Band 6) to deliver expert palliative care in patients’ homes and community settings across East Somerset. You will work autonomously within a multidisciplinary team.

The role emphasizes education, service development, and quality improvement, with opportunities to contribute to teaching and professional development while upholding our values of care and compassion.

Qualifications

• First Level Nurse qualification with Band 6 level experience.
• Clinical experience in palliative care.
• Experience facilitating Learning and Practice.
• Excellent communication/interpersonal skills.
• Ability to work independently and manage a caseload.
• Ongoing evidence of professional development.
• UK driving licence with access to a vehicle.

Responsibilities

• Deliver specialist palliative care across community services.
• Manage a caseload and provide assessments and support.
• Collaborate with GPs, district nurses and hospital staff.
• Provide clinical advice, support and education to colleagues.
• Contribute to service development and quality improvements.
• Participate in education and mentorship activities for partners.
