Job summary

Dorothy House invites applications for a Community Nurse Specialist, Band 6, delivering specialist palliative care across the community. This full-time post covers 37.5 hours per week, across East Somerset, with a supportive multi-disciplinary team.

You will manage a caseload of patients with life-limiting illness, support symptom management, and collaborate with GPs and hospital colleagues to coordinate high-quality care.

Responsibilities

• Managing a caseload of patients with life-limiting illness, providing specialist palliative care assessments, advice and support within the community
• Supporting patients and families with symptom management, advance care planning and complex decision-making
• Working collaboratively with GPs, district nurses, hospital colleagues and wider healthcare professionals to coordinate high-quality care
• Acting as a specialist resource for health and social care professionals, providing clinical advice, support and education
• Supporting the development of innovative community palliative care services and contributing to continuous quality improvement initiatives
• Participating in education, mentorship and training activities for colleagues, learners and partner organisations
• Promoting excellent multidisciplinary team working and ensuring care reflects the values and standards of Dorothy House
• Contributing to service development, audit, quality improvement and research activities where appropriate
