Job summary

Walsall Healthcare NHS Trust is seeking a Band 7 Community-based Palliative Care CNS to join the integrated specialist palliative care nursing team in Walsall. You will provide expert care across the borough, working with hospital and community colleagues to support patients with palliative needs.

Experience in palliative and cancer care, leadership qualities, and ability to work within MDTs are essential. The role includes weekends and on-call commitments, with NHS pension and potential staff

Qualifications

First level Registered Nurse Level 1
Degree in Palliative Care (or other relevant)
Recognised Teaching Qualification
Demonstrable experience in the acute or community setting
Knowledge of Palliative/Oncology care
Evidence of Continual Professional Development

Responsibilities

Provide specialist pralliative and end-of-life care in community and hospital settings.
Receive and manage referrals and discharge patients according to service criteria.
Undertake specialist assessments for moderate/complex needs and educate patients and carers.
Deliver care within the Palliative & End of Life Care Group.
Participate in regular MDT meetings and extended hours on-call rota.
