Job summary

Walsall Healthcare NHS Trust seeks a Band 7 Community Specialist Palliative Care CNS to work in the community, providing autonomous, advanced nursing care with an emphasis on end-of-life support. The role is part-time (30 hours weekly) with weekend work and on-call rotas.

The post offers NHS pension access and a supportive team within the Integrated Palliative Care Service, delivering high-quality care to patients in the borough and collaborating with hospital and hospice services.

Qualifications

• First level Registered Nurse Level 1
• Recognised Teaching Qualification
• Degree in Palliative Care (or other relevant)

Responsibilities

• Provide information, advice and support to community staff in palliative and end-of-life care.
• Promote a seamless package of care across illness phases.
• Receive and manage referrals and discharge patients per criteria.
• Undertake specialist assessments and educate patients, relatives and carers.
• Attend MDT meetings and participate in extended hours on-call rota.
