Job summary

Central and North West London NHS Foundation Trust seeks a Band 7 Clinical Nurse Specialist for the Symptom Control and Palliative Care team at UCLH. Qualifications

• Registered Nurse or Registered Paramedic with post-basic qualifications.
• Evidence of ongoing professional development.
• Recognised teaching qualification.
• Achieved or working towards a first degree.
• Extensive post-registration experience in the relevant specialty.
• Ability to work independently and as part of a team.
• Strong communication and time management skills.

Responsibilities

• Be expert clinician for patients and carers with complex palliative needs within UCLH.
• Independently manage a caseload of complex patients.
• Triage referrals and coordinate handover of referrals.
• Provide palliative care advice within multidisciplinary teams.
• Deliver assessments addressing advance care planning and end-of-life decisions.
• Serve as expert advisor on pain and symptom management.
• Assess and support bereavement needs and referrals.
• Provide education to staff and support to carers/families.
• Coordinate with external agencies to ensure optimal referrals and pathways.
• Participate in a 7-day rota with on-site coverage and on-call duties.
