Job summary

University Hospital Southampton NHS Foundation Trust is seeking a Band 6 Clinical Nurse Specialist to join the Supportive and Palliative Care Team. You will deliver expert supportive and palliative care to patients with complex needs across multiple clinical settings, working autonomously while collaborating with the multidisciplinary team.

Qualifications

• Registered nurse on relevant part of the NMC register
• Degree qualification or equivalent 120 credits at L6
• Post registration qualification within area of speciality or equivalent experience
• Completion of all Band 5 (preceptorship) competencies
• Provide evidence of supporting/ managing a range of learners in practice
• Provide evidence of supporting colleagues to promote effective learning in practice
• Extensive post registration experience within specialist area
• Knowledge of current issues within the acute health service
• Understanding of personal accountability
• Able to critically review and communicate clinical information

Responsibilities

• Providing specialist assessment, advice and personalised care planning for patients with palliative and supportive care needs across the Trust
• Coordinating care and ensuring effective communication between patients, families and the multidisciplinary team
• Supporting decision-making and advance care planning to help patients receive care that aligns with their goals and preferences
• Working collaboratively with hospital, community and primary care services to facilitate smooth transitions of care, minimise delays and support timely discharge whenever appropriate
• Providing education and support to healthcare professionals to enhance knowledge and promote high-quality palliative and end-of-life care across the organisation
• Contributing to service development and quality improvement initiatives to continually improve patient care and service delivery
